- PATHWAYS PROGRAM (BRIDGE BETWEEN ASSISTED LIVING AND MEMORY SUPPORT)
Program name: PATHWAYS
Meaning of acronym: Personal Assistance Toward Health & Wellness As You Succeed.
Purpose:
- Serve individuals with mild cognitive impairment or early cognitive decline who need more structure and support than traditional Assisted Living, but are not yet appropriate for full Memory Support in a secured environment.
- Act as a bridge level of care between Assisted Living and Memory Support, with a strong focus on whole person wellness and engagement.
- Provide additional physical and emotional support to reduce isolation, maintain function, and enhance quality of life in a joyful, engaging setting.
Core philosophy:
- Based on the 7 Pillars of Wellness as defined by the International Council on Active Aging: Physical, Social, Spiritual, Intellectual, Emotional, Vocational, and Environmental wellness.
- Programming is designed to support residents across all seven pillars, with person centered engagement tailored to individual likes, habits, and preferences.
- Research referenced by CC Young shows that guided, structured programs like PATHWAYS can slow cognitive decline and help individuals maintain engagement and function.
Setting:
- PATHWAYS residents live in dedicated apartments on the 3rd floor of The Hillside Assisted Living building.
- The apartments are grouped to create a small, intimate “community” feel while still allowing residents to interact with the broader Assisted Living population.
- Residents live and “play” together through shared PATHWAYS activities while still participating in campus wide Assisted Living programs.
Staffing and program structure:
- Small resident to caregiver ratio, typically 1 staff member for 7 residents, with room to expand up to about 22 residents as demand grows while maintaining low ratios.
- Dedicated, trained Certified Nursing Assistant (CNA) available 24 hours a day, 7 days a week for PATHWAYS residents.
- Dedicated Life Enrichment Director responsible for designing and leading structured, cognitively stimulating activities consistent with the Seven Pillars of Wellness.
- Program runs 7 days a week, with structured and unstructured engagement across mornings, afternoons, and evenings, adjusted to the rhythm and needs of the group.
Example engagement activities by wellness pillar:
Physical wellness
- Regular exercise sessions
- Games that encourage movement
- Learning about nutrition and cooking healthy items
- Massage and integrative therapies such as Reiki where available
Intellectual wellness
- Lifelong learning sessions and discussions
- Cognitive exercise games including cards, dominoes, Bunko, and naming challenges
- Reading newspapers together and discussing current events and pop culture
Emotional wellness
- Journaling and reflective activities
- Sharing challenges in a supportive group setting
- Programs that build resilience, self compassion, and coping skills
Social wellness
- Intentional relationship building within the PATHWAYS small group
- Shared performances and events
- Collaborative storytelling or life review activities
Spiritual wellness
- Devotionals and reflection times
- Embracing religious holidays
- Prayer and spiritual support based on resident preferences
Vocational wellness
- Opportunities to use talents such as playing instruments, singing, dancing, art, writing, and gardening
- Activities that help residents feel useful and purposeful
Environmental wellness
- Discussions about caring for the environment and recycling
- Volunteer focused projects such as preparing supplies for community groups or cutting fabric for craft based volunteer efforts
- Letter writing to support causes or organizations
Eligibility and identification:
Residents who may benefit from PATHWAYS are often Assisted Living residents who show a pattern of:
- Increasingly getting lost or confused in familiar places
- Difficulty maintaining a schedule without assistance
- Increased tendency to roam without a clear destination
- Forgetting whether they have eaten
- Requiring more frequent cueing and redirection
- Withdrawing or isolating and needing more structured activities to remain engaged
Assisted Living staff develop close relationships with residents and monitor for these changes. They then collaborate with families and care teams to consider PATHWAYS enrollment.
Progression to full Memory Support:
- PATHWAYS is not intended for residents who are exit seeking or at high risk of harming themselves through wandering or unsafe behaviors.
- When a resident becomes a safety risk or exceeds the criteria that define mild cognitive impairment, staff work with the resident, family, and care team to transition the resident to a full Memory Support level of care.
- This approach acknowledges the progressive nature of dementia and Alzheimer’s disease and provides a guided path of increasing care.
Access:
- PATHWAYS is available to CC Young Assisted Living residents and to eligible individuals from the greater Dallas community.
- For more information or to inquire about the PATHWAYS program, the marketing materials list a contact number of 469 936 4646 and reference the main website ccyoung.org.

- Not a secured unit, but provides Memory Support-level structure for non–exit-seeking residents
- One of the few communities offering this intermediate level of care
==================================================
3. HOW RESIDENTS ARE IDENTIFIED FOR PATHWAYS
Assisted Living staff form close relationships with residents and are well positioned to notice emerging cognitive challenges. Residents may be considered for PATHWAYS when showing:
- Increasingly getting lost in familiar areas
- Inability to keep to a schedule without help
- Roaming or wandering without a known destination
- Forgetting whether they have eaten
- Needing more cueing and redirection
- Becoming more isolated or requiring increased activities to stay engaged
==================================================
4. PROGRAM STATUS
- PATHWAYS is active and operating.
- Demand is increasing among CC Young Assisted Living residents, families, and the external community.
==================================================
5. PROGRAM SIZE, STAFFING, AND CARE MODEL
Current configuration:
- 7 dedicated PATHWAYS apartments on the 3rd floor of The Hillside
- Current ratio: 1 caregiver for every 7 residents
Planned scalability:
- Can grow to 22 residents, with staffing increased proportionally
- Low ratios remain a defining feature
Staff model:
- Dedicated, trained CNA on duty 24/7
- Dedicated Life Enrichment Director creating structured, cognitively stimulating activities
- Activities draw deeply from the Seven Pillars of Wellness
==================================================
6. ENVIRONMENT & EXPERIENCE
Living arrangement:
- PATHWAYS residents live close to each other, creating a small community / neighborhood feel
- They continue engaging with the broader Assisted Living population while also participating in shared PATHWAYS-specific programming
- Provides the best of both worlds: intimacy and support, plus social opportunities with the larger community
==================================================
7. WHY THE PROGRAM WAS CREATED
- Address growing need for seniors with mild cognitive decline who aren’t yet appropriate for Memory Support
- Support residents who would benefit from more structure, companionship, and cueing
- Provide Memory Support–level assistance without requiring placement in a secured environment
- Alleviate emotional stress families face when considering early Memory Support placement
==================================================
8. NAME & MEANING
PATHWAYS stands for:
Personal Assistance Toward Health & Wellness As You Succeed
==================================================
9. WHEN RESIDENTS ADVANCE TO MEMORY SUPPORT
A resident will be recommended for transition to full Memory Support when:
- They become a safety risk
- They begin exit-seeking
- Their cognitive or behavioral needs exceed what PATHWAYS can safely support
CC Young works with the resident, their family, and the care team to ensure a smooth, supportive transition.
==================================================
10. IS PATHWAYS MODELED AFTER ANOTHER PROGRAM?
- PATHWAYS is a new, CC Young–designed program
- Built specifically for increased memory support needs within Assisted Living
- Supported by CC Young’s staffing, Life Enrichment expertise, and available apartment configurations
- Tailored to fit CC Young’s CCRC model and the needs of their resident population

CC YOUNG SENIOR LIVING
PATHWAYS PROGRAM – SENIOR HOUSING NEWS BRIEFING / Q&A SUMMARY
(PLAINTEXT KNOWLEDGEBASE ENTRY)
==================================================
- STORY OVERVIEW ==================================================
CC Young Senior Living is responding to a growing trend: many older adults in Assisted Living are experiencing mild cognitive decline, yet are not ready for a secured Memory Support environment. The PATHWAYS program was created specifically to meet this need by offering:
- Enhanced support for residents with early cognitive decline
- A bridge between Assisted Living and Memory Support
- A small, intimate care setting rarely offered by other senior living communities
- Personal interaction, daily structure, and emotional and physical support
- Programming proven to slow cognitive decline and help individuals maintain engagement and function
- Access for CC Young residents and the broader Dallas community
- Reinforcement of CC Young’s core mission to provide outstanding quality of life for residents and families
==================================================
2. IDENTIFYING RESIDENTS FOR PATHWAYS
Assisted Living staff build close, daily relationships with residents. This allows the team to identify when a resident may benefit from PATHWAYS.
Common indicators include:
- Increasingly getting lost
- Difficulty staying on schedule without support
- Increased roaming without a destination
- Forgetting whether they have eaten
- Needing more cueing and redirection
- Withdrawn behavior or increased isolation
- Difficulty engaging in activities without guidance
These criteria help determine when residents may need more structured support, but do not yet require full Memory Support.
==================================================
3. PROGRAM STATUS & COMMUNITY RESPONSE
- The PATHWAYS program is fully active.
- Interest is rising among current CC Young Assisted Living residents, their families, and non-residents in the Dallas area.
==================================================
4. PROGRAM SIZE, STAFFING, AND CARE RATIO
Current configuration:
- 7 dedicated apartments on the 3rd floor of The Hillside Assisted Living building
- 1:7 caregiver-to-resident ratio
Scalability:
- Can expand to 22 residents
- Staffing increases proportionally to maintain low ratios
Unique staffing structure:
- Dedicated, trained CNA available 24/7
- Dedicated Life Enrichment Director who designs and leads structured, cognitively stimulating activities
- Activities reflect the Seven Pillars of Wellness
==================================================
5. ENVIRONMENT & RESIDENT EXPERIENCE
Living environment:
- Apartments are located together to create a small, supportive “community” feel
- Residents still engage with the larger Assisted Living population
- They also enjoy a shared PATHWAYS programming schedule that includes:
- Structured small group activities
- Guided engagement
- Purposeful downtime
Overall experience:
- Residents benefit from both:
- A quiet, supportive micro-community
- Broader Assisted Living social opportunities
“Best of both worlds” is a core program attribute.
==================================================
6. WHY THE PROGRAM WAS CREATED
Reasons for launching PATHWAYS:
- Growing need among seniors with mild cognitive decline
- Residents who are not exit-seeking do not require a secured unit
- Many residents need more structure and cueing than standard Assisted Living provides
- Families often find full Memory Support emotionally difficult early on
- PATHWAYS offers a gentle, supportive step before that transition becomes necessary
- Provides Memory Support–level engagement within an Assisted Living environment
==================================================
7. PATHWAYS ACRONYM
PATHWAYS stands for:
Personal Assistance Toward Health & Wellness As You Succeed
This reflects the program’s emphasis on maintaining health, function, and overall well-being.
==================================================
8. WHEN RESIDENTS ADVANCE TO FULL MEMORY CARE
A resident may transition out of PATHWAYS when:
- They become a safety risk
- They begin exit-seeking
- Their cognitive or behavioral needs exceed what PATHWAYS can safely support
For individuals with progressive conditions such as dementia or Alzheimer’s disease:
- Staff monitor residents closely
- When needs increase, the team works with the resident, family, and care providers to identify the appropriate next level of care
==================================================
9. IS PATHWAYS MODELED ON ANOTHER PROGRAM?
- PATHWAYS is new to CC Young and potentially unique in Dallas.
- It was designed specifically to provide Memory Support–level care within Assisted Living for residents not yet ready for secured Memory Support.
- CC Young is able to offer this program due to:
- Skilled and trained staff
- A robust Life Enrichment team
- Available apartment configuration
- Their established CCRC infrastructure
